Asthma Program

EMP has developed a program based on The National Institute of Health (NIH) guidelines for Asthma Care. Our Asthma Program promotes optimum asthma control in patients with poorly controlled asthma. Patient are assessed and where appropriate, referred into the program, our program provides patients with an individualized treatment plan, educational materials and encourages fewer visits to hospital emergency visits.

Diabetic Program

The goal of our Diabetic Program is to empower patients with the necessary tools to take the actions needed to control their diabetes. Diabetes is a chronic illness that requires continuing medical care and patient self-management, our program provides coaching, patient involvement, and educational materials used to help patient with diabetes successfully manage their diabetes, remove barriers and improve outcomes thereby reducing cost.
